From waiting lists to weekend impact: How high-volume dermatology clinics transformed access in a London trust
Long waits in dermatology can delay diagnoses, increase patient anxiety, and strain services already under pressure. In late 2023, an NHS Trust in London partnered with 18 Week Support (18WS) to deliver a targeted weekend model designed to restore timely access and reduce backlogs.
The result was a high-throughput approach that combined outpatient clinics with surgical theatres – accelerating patient pathways without compromising on safety or quality.

The vision
The challenge was clear: reduce long waits, including 52-, 65- and 78-week breaches, while preserving core weekday capacity. With backlogs continuing to grow and weekday clinics at capacity, a weekend solution was essential.
From August 2024, 18WS supported:
• Six weekend dermatology clinics – covering both General and Two-Week-Wait (2WW) patients
• Six weekend theatres – mainly for Minor Ops (MOPs) and See & Treat lists
Patients were triaged in advanced and seamlessly moved between clinics and theatres. In some cases, patients seen in 2WW clinics on Saturday were treated in theatre the very next day — drastically accelerating care timelines.
The results
• 3,842 patients treated in just seven months
• 2,642 outpatients seen
• 1,200 surgical procedures delivered
• 72 patients seen on average per day
“Having surgery scheduled just days after my initial appointment was a huge relief. I felt seen – and sorted – faster than I expected.”
— Patient feedback
Why it worked
• Integrated delivery: Clinics and theatres operated in tandem, allowing immediate escalation to surgery
• Efficient triage: Every patient was booked into the appropriate setting to maximise clinic and theatre slots
• Governance-led care: All clinicians worked under Trust protocols, with oversight built in from day one
• Workforce flexibility: Voluntary or rotational weekend staffing helped maintain morale and resilience
Built to last
What began as a short-term recovery initiative evolved into a scalable model for delivering high-quality elective dermatology care.
With measurable impact on access, performance, and patient experience, the partnership showcases how strategic insourcing can drive long-term transformation – without compromising governance, safety, or outcomes.
